2. Pay Dues for Multiple Members Under One Hospital

Hospitals can now submit bulk payment for up to ten member applications at once using our updated “Pay Membership Dues for Hospitals” form.

Select the number of members under the “Individual Member Names” header, then fill in the full name for each applicant.

NOTE:  You or the applicant(s) MUST also submit INDIVIDUAL APPLICATIONS for each name listed on this form  —  www.sfvma.net/join
